Medical Release Form: 

Every Student MUST have o Medical Release Form on file in the Youth Office in order to participate in Youth Activities. You may pick up a copy in the Youth Gathering Room or download here.  Please note:  This form is DOUBLE-SIDED and must be printed on ONE sheet of paper.  It also must be notarized ... we can notarize it here at the church for you.  Click here MEDICAL RELEASE.doc

Behavior covenant Form: 

As we strive to create a safe place for Students and to share Christ in the world, we have broad guidelines for how we will treat each other, and other’s property,

Each Student will need to have a copy of this in the Youth Office signed by both Student and Parent.

Senior High Ski Retreat 2008
